Obesity Management Foundations 
for the Primary Care Clinician

Core Program


The American Association of Nurse Practitioners® (AANP) is pleased to offer select continuing education (CE) activities and resources for nurse practitioners (NPs) and other advanced practice providers working in obesity management. The Obesity Management Foundations for the Primary Care Clinician program provides advanced practice clinicians online access to a convenient, comprehensive and foundational obesity management program.

Consisting of nine sessions, this program enables NPs and other clinicians to increase their understanding of obesity as a chronic disease, gain a blueprint for the evaluation of obesity management and develop treatment modalities and interventions for patients with the disease of obesity. Participants may earn 9 contact hours (CH) of CE credit by completing all nine activities in this program.

By completing this Obesity Foundations Program, NPs and other clinicians can increase their understanding and foundational education on evidence-based methods for assessing, diagnosing and managing obesity within a clinical environment. The material suits NPs, physicians, physician assistants, trainee clinicians and other health care professionals who treat patients with the disease of obesity.

Upon completing all nine activities, participants will receive the Obesity Management Foundations for the Primary Care Clinician Certificate. The certificate of academic achievement demonstrates to employers, workers and peers their commitment as a clinician in obesity care. Participants who have completed the program and received their certificate will also be interested in an Obesity Across the Lifespan companion program to be released later this year.


 



Recognizing Obesity as a  Chronic Disease - Part 1

This session will discuss the fundamental components of obesity as a chronic disease state. We will define obesity, discuss the many complexities of obesity as a disease and address how to approach the treatment of obesity in a comprehensive way to help patients obtain a healthier weight. 
Objectives:
  • Define obesity.
  • Discuss the etiology of obesity.
  • Discuss chronicity of obesity disease.
  • Discuss the prevalence of obesity in different populations and subgroups.



Recognizing Obesity as a Chronic Disease - Part 2
 
Lori Wenz, MSN, AGNP-C, BC-ADM, FOMA
This session will review the physiological and psychological health risks associated with obesity, and will examine the impact that obesity has on society.  The effect of weight bias and stigma on people living with obesity will be reviewed, and the use of people first language will be discussed.  The importance of early diagnosis and treatment of obesity will be examined, and the chronic care model will be presented as appropriate approach to long term disease management.
Objectives:
  • Describe health risks of obesity.
  • Discuss weight bias/stigma and potential effects.
  • Discuss importance of timely diagnosis and treatment for the disease of obesity.



Obesity Physiology

What are the clinical realities of calories in versus calories out? What is the obesity "setpoint"?  How does obesity cause metabolic disease? This session will discuss practical clinical applications of obesity physiology and pathophysiology.
Objectives:
  • Describe components involved in normal body energy regulation.
  • Discuss the function of the adipocyte in energy regulation. 
  • Discuss how weight and appetite are regulated.



Obesity Pathophysiology

This session will further expand on the content from Session 3 and delve deeper into the pathophysiology of obesity. During this session, we will explore the foundational mechanisms of pathophysiology while also analyzing the roles played by the microbiome, sleep patterns, and body fat distribution in influencing both energy dysregulation and obesity.
Objectives:
  • Describe the mechanisms of compromised energy regulation. 
  • Discuss energy dysregulation complications.
  • Discuss the contribution of the microbiome, sleep, and body fat distribution in energy dysregulation.
  • Discuss the role of genetics in energy dysregulation.



Assessment and Diagnosis

This session will provide guidance for utilizing the current guidelines and algorithm to treat patients with pre-obesity (overweight) and obesity. A review of how obesity fits into a chronic care model will be discussed. Using case studies this session will demonstrate using screening tools and measurements to make a diagnosis and provide staging of the chronic disease of obesity.
Objectives:
  • Describe the chronic care model and its relationship to obesity care.
  • Discuss evidence-based guidelines for the disease of obesity.
  • Discuss screening methodologies for overweight and obesity.
  • Discuss the staging of obesity disease.



Evaluation and Overview of Obesity Management

This session will provide the participant with a blueprint for completing an assessment for a patient with obesity including a weight history, physical examination pertinent to obesity and the diagnostic workup. It will then move into important differential diagnosis and the conditions that obesity can cause and worsen.  Finally, the session will end with the importance of using shared decision making in creating a plan of care using the information gleaned from the assessment to individualize the treatment plan.

  • Discuss considerations of the diagnostic work-up for the patient with obesity.
  • Discuss obesity related conditions.
  • Discuss shared decision-making process.



Treatment Modalities and Interventions - Part 1

Intensive Lifestyle Therapy (ILT) is a foundational component of healthcare for people with obesity. In this course, the treatment modalities and interventions for obesity (part 1), we will present the pillars of ILT that can be a part of the tool kit of every primary care provider when working with a person who has obesity. We will begin with a review of patient-centered care incorporating a shared decision making model based on evidence. Next, since obesity is a vastly multi-faceted disease, treatment and interventions should also be multi-faceted. Therefore, we will discuss behavioral, psycho-social, environmental, nutritional, and activity factors of ILT. During this course, we will also review barriers to factors of ILT that may prevent success and compliance and discuss ways to find solutions for barriers that cannot be modified. Finally, we will review strategies to facilitate healthy behavior change and on-going support of our patients.
Objectives:
  • Describe components of Intensive Lifestyle Therapy (ILT).
  • Discuss nutrition options included in ILT.
  • Discuss potential activity barriers for the patient with obesity.
  • Describe 3 communication strategies to aid development of ILT plan.



Treatment Modalities and Interventions - Part 2 (Pharmacotherapy)

Pharmacotherapy is one of the four pillars of evidence-based obesity treatment. Anti-obesity medications facilitate weight reduction that improves health. They are most effective when they are used as part of a comprehensive obesity treatment plan that includes nutritional therapy, physical activity, behavior modification, and bariatric surgery if indicated. Despite their safety and efficacy, only 1-3% of those eligible receive a prescription for FDA approved anti-obesity medications. This presentation will help you unlearn any outdated or biased beliefs about these medications and learn the latest science on how you can use them safely and effectively. Real-life clinical scenarios will help you learn how to select, initiate, titrate, and monitor these medications. Tips and strategies for personalized medication selection based on complications, co-existing conditions, symptoms, patient preference, cost, and insurance coverage will be provided. 
Objectives:
  • Describe 3 classes of weight promoting medications.
  • Discuss the difference between FDA categories of "on-label" and "off-label".
  • Identify the available FDA-approved anti-obesity medications, including mechanism of action, contraindications, interactions and adverse effects.
  • Describe how to initiate, titrate and monitor response to medications, including criteria for continuation and discontinuation.



Treatment Modalities and Interventions - Part 3

This session will discuss the role of devices, endoscopic procedures and metabolic and bariatric surgery in the treatment of obesity.  Each treatment modality will be described and considerations for patient selection will be reviewed.  The expected outcomes for each modality will be presented.   
Objectives:
  • Describe definition of device.
  • Discuss difference between device and endoscopic procedure.
  • Describe 2 most common metabolic and bariatric surgery (MBS) procedures in adults.
  • Discuss outcomes from MBS procedures.

    Tiffany N. Lowe Clayton, DO, DABOM, MHPE (a.k.a Dr. TLC), is an assistant professor and director of clinical education at Campbell University School of Osteopathic Medicine in Lillington, North Carolina. She is also an obesity medicine specialist at WakeMed Bariatric Surgery and Medical Weight Loss, with practices in both Cary and Fuquay-Varina, North Carolina. Additionally, she is the Founder and CEO of the Institute of Transformational Health and Wellness, Inc. in Wendell, North Carolina. She earned her medical degree in 2000 from Rowan University School of Osteopathic Medicine in Stratford, New Jersey, where she also completed an internship and residency in family medicine in 2003. A consummate teacher at heart, Dr. TLC is the author of the best-selling book, “A Woman’s Guide to De-Stress for Success: 10 Essential Tips to Conquer Stress and Live At Your Best.” In her personal life, Dr. TLC is a devoted wife, mother, and “Glamma.” She enjoys spending time with her family as well as traveling, singing and recreational boxing.

    Lori A. Wenz is a nationally board-certified nurse practitioner in adult gerontology and advanced diabetes management. She is a Fellow of the Obesity Medicine Association (OMA), holds a Certificate of Advanced Education in Obesity Medicine from the OMA and an Assessment Based Certificate from the American Society of Metabolic and Bariatric Surgery (ASMBS). Ms. Wenz served as a Column Editor for Bariatric Times from 2023-2024, and currently serves as Associate Editor for Obesity Pillars. She has presented and published on a variety of topics and was a contributing author of the OMA Clinical Practice Statement on stress, psychiatric disease, and obesity (Obesity Pillars, 2022) and editor of the chapter on Obesity in the 2024 revision of Lewis’s Medical-Surgical Nursing 13th Edition (Elsevier). Ms. Wenz is the owner, founder, and nurse practitioner at Western Colorado Weight Care in Grand Junction, Colorado. Prior to opening her obesity specialty clinic, Ms. Wenz treated obesity in the settings of family practice, endocrinology, and a comprehensive obesity treatment center. She has dedicated her career to partnering with patients to manage weight, improve health and transform lives.

    Dr. Harold Bays is the Medical Director and President of the Louisville Metabolic and Atherosclerosis Research Center and Clinical Associate Professor at the University of Louisville School of Medicine. He has served as a Principal Investigator for over 600 clinical trials. Dr. Bays is Board Certified in Internal Medicine and Endocrinology and is a Diplomate of the American Board of Obesity Medicine. Dr. Bays has authored/co-authored over 300 peer-reviewed scientific manuscripts and book chapters and has authored/coauthored and/or presented hundreds of abstracts at national and international scientific conferences. He has authored/co-authored scientific consensus statements, position statements, and clinical practice statements.

    Dr. Bays originated the term "adiposopathy" ("sick fat"), which helps define the adverse metabolic consequences of obesity-driven dysfunctional adipose tissue. Dr. Bays is Editor-in-Chief of Obesity Pillars® (the official journal of the OMA) and Chief Science Officer of the OMA. Dr. Bays was awarded the Obesity Medicine Clinician of the Year Award (2016), the Committee Leadership Award (2014), and the Dr. Peter G. Lindner Award (2019).

    Dr. Angela Golden, Chief Clinical Officer and Family Nurse Practitioner at NP From Home, LLC, in Munds Park, Arizona and NP Obesity Treatment Clinic in Flagstaff, Arizona. Dr. Golden earned her doctoral degree with an emphasis on health policy and evidence-based practice at Arizona State University in Phoenix, Arizona.

    Dr. Golden is a member and fellow of American Association of Nurse Practitioners, where she served as past president and founding co-chair of the Obesity Specialty Practice Group/Community. She is a member and Fellow of the Obesity Medicine Association and served as the Chair of the CME overview committee. She is a member of the Arizona Nurse Practitioners Council (Chapter AZNA) and The Obesity Society where she serves as the Chair for the TOS Clinical Practice Section.

    Dr. Golden has presented at conferences regionally, nationally, and internationally. She is the author of several book chapters and books, including Treating Obesity in Primary Care. Her research has been published in periodicals including the Journal of the American Association of Nurse Practitioners, Nurse Education Today, Obesity, Population Health Management, and Postgraduate Medicine. Dr. Golden is the recipient of several honors and awards, including the 2024 AANP Sharp Cutting Edge Award, 2023 America’s Top NP Award, and 2021 Steelman-Seim Educator Award for Excellence in Academics from the Obesity Medicine Association.

    Christy Davis, Family Nurse Practitioner, received her MSN (2015) and DNP (2019) from Mississippi University for Women. She has been a nurse since 2007 and has 9 years of experience working as an NP primarily in Obesity medicine and endocrinology. She earned an endocrinology specialty certification from Duke University and a Certificate of Advanced Education in Obesity Medicine. She is a recipient of the 2023 AANP Nurse Practitioner State Award for Excellence.

    Dr. Davis co-founded the Mississippi Obesity Society and is currently serving as co-chair and political action lead. She authored a MS State House Resolution to support reducing cardiovascular risk to Mississippians including addressing racial and economic disparities in obesity treatment. She successfully advocated for changes to Mississippi legislation allowing the state Medicaid addition of anti-obesity medications.

    Dr. Davis is employed as a NP at Metabolic Medicine of Mississippi in Jackson and as a FNP Graduate Instructor at Mississippi University for Women.

    Rebecca Graves is Associate Professor and Director of Special Projects and Evaluation at the University of South Alabama (USA) College of Nursing and a Fellow of the American Association of Nurse Practitioners. She is a nurse scientist, a family nurse practitioner, and nursing faculty. Since 2016, she has served on an interprofessional team to bring culinary medicine to USA and has conducted culinary medicine classes for students. She is part of an interprofessional research team at USA studying health promotion and prevention and treatment of obesity in multiple vulnerable populations. Dr. Graves has worked with a community partner to examine experiences of low-income senior citizens with food insecurity during the COVID-19 pandemic. She has served AANP as a co-chair of the Obesity Specialty Practice Group since 2021. Dr. Graves loves spending time with her family, friends, and pets and enjoys camping, fishing, hiking, and being outdoors and on the water.

    Sandra Christensen is the founder and Chief Clinical Officer of Integrative Medical Weight Management in Seattle, Washington and has specialized in obesity management since 2005. She is board certified as a family nurse practitioner and holds the Certificate of Advanced Education in Obesity Medicine from the Obesity Medicine Association.

    Ms. Christensen is a Master Fellow of the Obesity Medicine Association and a Fellow of the American Association of Nurse Practitioners. She serves on the Board of Trustees for the Obesity Medicine Association and is the President of the Washington Obesity Society. Ms. Christensen is an associate editor of the journal Obesity Pillars and authors peer-reviewed articles on the topic of obesity. She authored the book, “A Clinician’s Guide to Discussing Obesity with Patients” published by Springer. Ms. Christensen was named the 2022 Obesity Medicine Association Clinician of the Year and received the Committee Leadership Award and the Dr. Raymond E. Dietz Meritorious Service Award from the Obesity Medicine Association.

    Ms. Christensen is a frequent speaker for the Obesity Medicine Association, AANP, Washington Obesity Society, as well as numerous other educational events and programs.
