Overview This educational activity is designed to empower nurse practitioners to improve adult immunization rates. It highlights the critical role of the patient-centered medical home (PCMH) model in promoting life-course vaccination, addressing systemic barriers, dispelling myths, and equipping clinicians to serve as vaccine champions in their practice settings. Through practical strategies and real-world implementation tools, the program supports NPs in integrating equitable, efficient, and sustainable adult vaccination programs into diverse clinical environments. Objectives
Review the current adult vaccination landscape, including the latest recommendations and trends in vaccine uptake.
Describe how increasing vaccination uptake across the lifespan can benefit individual patients, medical homes, and the public at large.
Discuss the ways in which medical home environments can equitably support the full spectrum of vaccination recommendations across the lifespan.
Identify barriers that impede the medical home in providing access to vaccines across the lifespan.
Integrate evidence-based approaches to improving vaccination adherence and uptake among patient populations in medical home settings.
Identify tools and resources that are available to providers and personnel that enable continuous learning and performance improvement in vaccine access and vaccine uptake.
