Overview Hierarchical Category Condition (HCC) coding is an essential component to NP practice although many NPs lack training in the clinical and financial implications of HCC coding. The Centers for Medicare and Medicaid (CMS) updated HCC Version 24 to V28. Learning the impact of Version 28 HCC scores on NP practice is critical, especially for those who participate in value-based payment models. The course highlights the impact of HCC coding, reimbursement, and management of complex patient care conditions using patient case scenarios to list required documentation. Objectives
Define HCC and Risk Adjustment Factor.
Describe CMS-HCC Version 28 coding changes and updates.
List requirements for specific documentation of patient conditions.
Explain how to document HCC codes using patient case scenarios.
