Overview This presentation offers nurse practitioners evidence-based, practical strategies for assessing and addressing the oral health needs of older adults in primary care. It highlights the strong connection between poor oral health and the progression of chronic diseases, offering guidance on incorporating oral health assessments into routine patient visits. The session will also examine common barriers to oral care, such as financial constraints, access issues, and gaps in oral health education. Special focus is given to the complexities of managing oral health in older adults with chronic conditions, highlighting the importance of symptom monitoring, medication management, caregiver education, and enhancing provider skills in preventive oral health care.

- Describe three ways in which poor oral health contributes to the progression of chronic diseases in older adults.
- Apply evidence-based oral health screening and patient education strategies to older adults.
