    Overview
This activity provides an in-depth overview of essential billing, coding, and documentation practices for clinicians involved in wound and ulcer management. Participants will learn how Medicare coverage is determined, how to apply ICD-10 and CPT codes correctly, and what documentation elements are required to support medical necessity and withstand audits. The session reviews national and local coverage determinations, common coding pitfalls, new 2026 ICD-10 updates, required components of procedure notes, and proper reporting of cellular, acellular, and matrix-like products (CAMPs/CTPs). Emphasis is placed on avoiding fraud-related risk, understanding payer-specific requirements, and using standardized documentation to ensure accurate reimbursement and compliant wound care practice.

    Objectives
    • Define how coverage is determined for wound care.
    • Review coding and documentation requirements.
    • Discuss the current landscape with applying cellular, acellular and matrix-like products (CAMPs) and the targeting of Nurse Practitioners by bad actors in the space.
