    Overview
This session will provide an evidence-based overview of the management of acute respiratory failure in adult patients. We will begin with a review of the two primary types of respiratory failure—hypoxic and hypercapnic—including common etiologies, underlying pathophysiology, and clinical presentations. The focus will then shift to high-flow nasal cannula (HFNC) systems such as Optiflow and Vapotherm, as well as noninvasive mechanical ventilation (BiPAP/CPAP). I will cover the physiology, mechanisms of action, and clinical indications for each modality, followed by best practices for initiating, managing, and weaning support. Recent evidence and guideline updates published in CHEST on HFNC and noninvasive ventilation will be highlighted. The session will also address how to recognize clinical deterioration and when escalation of care is warranted. Designed for acute care nurse practitioners, this presentation will offer practical strategies and clinical pearls to support bedside decision-making and optimize respiratory support for critically ill patients.

    Objectives
    • Differentiate between hypoxic and hypercapnic respiratory failure based on clinical presentation, underlying pathophysiology, and common etiologies.
    • Describe the mechanisms of action, and clinical indications for high-flow nasal cannula (HFNC) and noninvasive ventilation (NIV) in adults with acute respiratory failure.
    • Apply guideline-based strategies to initiate, titrate, and wean noninvasive oxygenation modalities, and recognize clinical signs indicating need for escalation.
