    Obesity Care through the Lifespan
    Pediatrics Evaluation and Assessment

Sharon Karp, PhD, APRN, CPNP-PC, FAANP
Nancy Browne, MS, PPCNP-BC, FOMA, FAANP, FAAN
Pediatric obesity is a multifaceted health issue that demands a nuanced and informed approach from healthcare providers. This engaging presentation will delve into key considerations that set obesity in children and adolescents apart from obesity in adults. A deeper understanding of these unique physiological factors, combined with current evidence-based guidelines, will empower healthcare professionals to offer more effective support to young patients and their families.
Initially, we will examine two critical physiological distinctions in pediatric obesity. Growing children experience dynamic hormonal fluctuations and metabolic shifts that significantly influence weight regulation. Additionally, the developing nervous system plays a pivotal role in controlling appetite and satiety signals, presenting differently in children compared to adults and requiring specialized clinical attention.

We will then review two influential evidence-based publications essential for guiding clinical practice. The American Academy of Pediatrics' clinical practice guideline provides a comprehensive framework for assessing, treating, and preventing pediatric obesity. Concurrently, the World Health Organization's report on ending childhood obesity outlines global strategies and policy recommendations essential for addressing this public health challenge. Lastly, we will highlight two clinical scenarios that should prompt consideration for genetic testing. Genetic conditions such as Prader-Willi syndrome and Bardet-Biedl syndrome can contribute to severe or early-onset obesity. Early recognition of these clinical patterns allows for timely genetic evaluation and personalized therapeutic strategies.

By exploring these critical nuances, healthcare providers can offer targeted, evidence-informed care, ultimately improving health outcomes and empowering young patients and their families on their journey toward wellness.
    Objectives:
    • Discuss 2 key concepts unique to pediatric population with implications for obesity care.
    • Discuss 2 publications giving evidence based pediatric obesity care guidance.
    • Describe 2 presentations suggestive of need for genetic testing in youth with obesity.

Sharon Karp, PhD, APRN, CPNP-PC, FAANP
    Lisa Watson, MSN, APRN, CPNP-PC
This presentation will provide an overview of key considerations in managing pediatric obesity, focusing on nutritional assessment, activity barriers, and psychosocial concerns. We will begin by discussing the essential components of nutritional assessment in the pediatric population. We will explore the importance of evaluating dietary intake, discuss barriers to dietary changes, and give practical strategies to promote healthy eating habits. We will address health-promoting measures of success that focus on overall health rather than weight. We will explore common concerns that hinder physical activity: psychological barriers, such as low self-esteem and body image issues, and environmental barriers, including access to recreational facilities and sedentary family habits. We will discuss practical strategies for promoting physical activity. Finally, we will explore psychosocial concerns faced by children and adolescents with obesity. We will highlight the effects of adverse childhood events (ACES) and bullying on obesity. We will also discuss the importance of screening for and preventing the onset of eating disorders. Lastly, we will discuss the risk of comorbid conditions, including depression and anxiety, and the importance of addressing these issues holistically.

By addressing these multifaceted aspects, healthcare providers can develop more comprehensive and effective interventions to support the long-term health and well-being of children and adolescents with obesity.
    Objectives:
    • Discuss 3 components of nutritional assessment in children with obesity.
    • Discuss 2 concerns that may be barriers to activity in children with obesity.
