Overview This course topic was initiated through the AANP Telehealth and Health Informatics SIG. Although initially started in Europe, it was almost 30 years later that the concept of Hospital at Home (HaH) surfaced in the United States. Despite interest from federal insurers and some large healthcare institutions, it would take another 2-3 decades and a world-wide pandemic to bring this concept to the forefront of healthcare delivery. Is it a healthcare delivery option that served its purpose – used only during the pandemic – or is it here to stay as a viable option for patients and healthcare entities alike; a potential disruptor of traditional acute healthcare delivery? This course will provide an overview of HaH – what it is and is not, its history, the Centers for Medicare and Medicaid Services (CMS) involvement, and its evolving role during the SARS CoV2 pandemic will be represented.
This course is delivered in three sections by three nurse practitioner experts at three different healthcare institutions at three different geographic locations in the United States, each are currently using HaH. Each program is similar yet different as it was initially molded by its patient population’s needs, providers input and waiver requirements.
Objectives
- Define patient types that are appropriate for a HaH program.
- Differentiate at least two reasons/rationale for HaH program origination.
- Identify at least 3 pearls or patient outcomes identified from HaH programs.
- A short question and answer period will occur with each speaker.
