Overview
Cardiovascular-Renal-Metabolic (CVRM) Diseases - also known as cardio-kidney-metabolic (CKM) syndrome - encompasses a constellation of conditions including cardiovascular disease (CVD), chronic kidney disease, and metabolic disorders such as diabetes and obesity which, combined, are the leading cause of death worldwide. This activity is designed to aid NPs who are pivotal members of the healthcare team to understand the pathophysiologic interplay of cardiovascular-renal-metabolic diseases, adopt guideline-driven strategies to mitigate risk and thoughtfully select therapies known to decrease the burden of disease across the CVRM spectrum, recognize ways in which social determinants of health (SDOH) impacts barriers to optimal CVRM care and identify ways in which new data and evidence-based approaches to comprehensive CVRM care may be swiftly integrated into individual practice.
Objectives - Implement evidence-driven and guideline-concordant strategies for recognizing and managing cardiometabolic disease manifestations.
- Evaluate cardiometabolic disease risk for individual patients using current assessment tools and methodologies.
- Address social determinants of health (SDOH) contributing to disparate cardiometabolic clinical outcomes and fragmented care.
- Utilize new strategies for a multidisciplinary approach to comprehensive cardiometabolic care, fostering collaboration among cardiology, nephrology, and diabetes specialists, emphasizing shared decision-making.
