Overview
The use of diabetes technology can be life-changing for the person living with diabetes. Much of diabetes care occurs in the primary care setting and members of the care team need to be skilled in discussing and initiating diabetes technology with their patients who are living with diabetes. Part 1 of this activity will inform and empower primary care providers to identify and implement diabetes technology for people with diabetes in the primary care setting. Part 2 of this activity includes interactive case-studies that will provide primary care providers with the opportunity to discuss, initiate and follow-up on the use of diabetes technology in patients with diabetes. The expert faculty will present case studies involving CGM, smart insulin pens and AID.
Objectives - Summarize common diabetes technologies and their benefits for people living with diabetes.
- Devise a plan to integrate diabetes technology into the primary care setting that includes shared decision-making, diabetes education, prescribing, coding/documenting and follow-up.
- Discuss available diabetes technology resources for the provider and for the person living with diabetes.
